I understand and agree to give Music 4 Kidz, its representatives and employees the right to take photographs/videos of my child.

I authorize Music 4 Kidz, its assigns and transferees to copyright, use and publish the same in print and/or electronically.

I agree that Music 4 Kidz may use such photographs/videos of my child for any lawful purpose, including for example such purposes as publicity, illustration, advertising, and Web content.
